This agreement is an ongoing agreement. Member agrees to purchase for a minimum of 12 consecutive months (unless otherwise specified above) the use of this Curves facility at the address listed above. Except for the fees outlined above, there is no additional fee for each visit to this Curves facility for the Member.

Member may put their membership on hold without any penalty in the event Member provides Curves with a doctor's notice of Member's physical inability to continue the use of this Curves facility. If the physical inability is a permanent sickness or physical incapacity, Member may terminate this agreement by giving Curves written notice of the termination and by supplying medical certification of the permanent sickness or physical injury, and a proportional refund of any unused fees less an administrator fee of RM50 will be made within 7 days of such written notice. For any other termination, Curves shall have the right to charge Member a termination fee, determined by the number of months of contract outstanding per Curves Malaysia policy guidelines.

Curves reserves the right to increase the monthly membership fee by at least a percentage rate equivalent to the then applicable CPI rate for Malaysia, as issued by the appropriate Authorities, from time to time.

The Personal Data Protect Act 2010 ("Act") requires Curves to inform you of your rights in respect of your personal information that is being processed or being collected to be further processed by us.  The Act also requires us to obtain your consent to the processing of your personal information.

By submitting this form, you hereby agree that by your providing Curves with your personal information, you have agreed to and consent to our using and to our continual use of your personal information for the following purposes (collectively "Purposes"): -

(a) to communicate with you;

(b) to process your monthly dues transactions;

(c) to respond to your enquiries;

(d) to conduct internal activities;

(e) to store your personal information in our database;

(f) other legitimate business activities of our company concerning you.

Further, if required for any of the foregoing Purposes, your personal information may be transferred to locations outside Malaysia or disclosed to our related corporations, licensees, business partners and or service providers, who may be located within or outside Malaysia.

I, the Member, certify that I am physically able to use all facilities and do hereby agree that Curves is not responsible or liable to me for any injury, accident or loss of personal property. I understand that I cannot transfer this membership to any other person. I do hereby release Curves and its employees from any claim or cause of action which may have occurred as a result of any medical problem known or unknown which I have knowledge presently or in the future. I verify no promises or guarantees, other than those written in this agreement, were made to me by this facility or its employees. I agree to follow Curves instructional guidelines and to cooperatively utilize the facilities with other members. Failure to do so may result in cancellation of my membership.
